Test Kit Request

You can request a self taken test kit online by completing your details below. The kit will be sent to the address you provide in an unmarked plain envelope.

The kit will contain specimen container, laboratory test request form, National Screening Programme leaflet, pre paid returns envelope and protective returns packaging.

When you have taken your sample, simply return it via any Royal Mail post box on the same day. Your results will be sent by the method you choose on the laboratory test request form.
